This notice is to make you aware of a new clinic policy that may affect your eye exam co-pay.
– Routine Exam: If no medical conditions are found, the exam is billed to your vision plan.
– Medical Exam: If you have a condition such as diabetes, glaucoma, or macular degeneration, the exam is billed to your medical insurance.
– Possible Costs: There may be co-pays or deductible fees based on your medical insurance plan.
